Adult Day Program (ADP) The Adult Day Program plays a key role in allowing people to remain living in the community as long as possible by optimizing their level of physical, social and emotional functioning. Program Funding This is a community-based grant funded program of the Interlake Eastern Regional Health Authority (IERHA).
Program Overview The Adult Day Program operates every Wednesday from 10am to 2:30pm in the main hall of the East Beaches Social Scene building. It provides recreational programs, both mental and physical, giving ADP clients socialization opportunities and peer support. It also provides respite and support to caregivers.
The ADP provides transportation within the catchment area to and from the program, nutritious snacks and lunch, education on health promotion activities and outings within the community.
Participant Eligibility Participation is based on an IERHA Home Care Case Coordinator simple assessment and referral. You do not need to have Home Care to qualify. A Home Care Case Coordinator will either come and visit you or phone you to assess that this program is the right fit for you.
Client considerations may include one or more of the following:
Limited social interaction
Physical or cognitive impairments
Able to participate in a group setting
Seeking positive social interaction
Unable to access recreational/social programs in their community/residence
Families/caregivers requiring respite
Function considerations:
Able to ambulate (with aids as needed) and transfer independently, or mobilizes with wheelchair but able to transfer on your own
Able to follow simple directions or instructions
Continent and/or independent in management with continence products
Require minimal to no assistance with medication management
Program Fees Participants pay a fee of $9.80 per day, includes transportation, snacks and lunch. These fees are paid directly to the Adult Day Program Coordinator.
